Charges for medical services are due and payable at the time services are rendered. Payment may be made by cash, check, Mastercard, or Visa.
If your account has not been settled in 180 days, it may be placed for collection and future services provided on a cash basis only.
If your insurance requires a deductible and/or co-payment, please be prepared to pay at the time services are rendered.
