The following information is necessary to facilitate the delivery of your healthcare. Please read over this carefully.
Your health insurance is designed to help you meet the cost of your medical care. Please inform the appointment secretary of your insurance coverage when you call to schedule an appointment.
Since insurance benefits are determined by your insurance plan, it is your responsibility to understand your insurance benefits.
Please be aware that many insurance plans require that you receive medical care from "in-network" providers. If you choose to receive medical care outside of network, your benefits will likely be reduced.
If your health insurance requires a co-payment, co-insurance, and/or deductible, you are responsible for payment at the time of your scheduled service. You are also responsbile for obtaining a referral or authorization, if required by your insurance plan. If you do not have health insurance, you are responsible for full payment at the time of service.
